Mother’s Day is fast approaching and I wanted to show you what my little class is working on to present to their mamas at our annual Muffins With Mom event this week.
I have seen a lot of FREE Printable All About Mom Questionnaires on Pinterest, but I really wanted mine to have a custom-look and have all the questions I liked best, so I put this one together (FREE download at the end of this post).
The kids were SO cute answering the questions about their moms.
I especially liked hearing their responses to, “I love my mom more than…” This little girl above said she loved her mom more than donuts. That is some serious love right there.
I also got a kick out of the “If I could buy my mom a gift, I would buy her…” Here is a sampling of some of the answers we got…
“A light-up bracelet.”
“A croissant.”
“A goat.”
We added a special handprint flower pot and photo of each child and then laminated it for years of memory-preserving preciousness.
If you would like to use this idea in your own classroom or Sunday School, find it below!

Cute idea, but I need an idea for our nursery…9 months old, can’t talk yet 🙁 Do you have any ideas for that age group?
Hi Penny! You could just do the flower pot with handprint!
How do we download the handprint flower pot and the petals?
This is ADORABLE!!! Pinning it!
This is so precious, Julie. Thanks for the cuteness overload.
This is so cute!! Thank you for this!! I love it!!
This is adorable but I was wondering if you had any suggestions on how to use this for my kiddos Nanas?
These really are the best questions!! Also: I just love your model’s outfit. ;o)
Do you have this in black and white, so my Kindergarten students could color their own after they are finished filling out the questionnaire?
Hi there! I am so sorry but I do not! 🙁
Do you have this with the Canadian spelling of ‘favourite’?
I agree with the black and white version. Would love if you could add one for next year!
Hi there, do you have a version for African American mommies???
I am so sorry but I do not! 🙁
I keep trying to download this but keep getting error messages. 🙁
Hi Jillian! I am so sorry! That stinks! Everything is working on my end – do you have the latest version of Adobe Reader? 🙂
Hello!
Do you happen to have a template for the flower pot and little flowers off of the fingers?
How do you make the flower pot ? What materials do I need for this project ?
Love the project! I was just wondering how you did the flowers at the top of the handprint. Is that a paint sponge, shaped like a flower? Or is that construction paper flowers that you pre-made? Thanks!
Love this so much! Any chance I could get a version of it for a Grandma?? This is who a little lives with… Thanks for your time.
Is there anyway you could send this to me that says Grandma???
Do you have the flowerpot printables? Thanks in advance!
Thanks for the adorable mommy questionaire!!!
Hi!
Do you have a black and white copy of the “My Mommy” worksheet? I hope you are having wonderful day!
Thank you so much,
Salma Waheed
It would really help if you could have a black and white version. Not all moms are Caucasian and have dark hair. I would love to use this but cannot in my classroom unless it was in black and white.
Can you do this with the Australian spelling for MUM please.
Do you have the My Mommy with a boy
Hi I was wondering if you can provide a copy of the questionnaire with ‘mum’ or ‘mummy’ instead? Thanks!
Hi, I loved it! but can I please have a black and white printout for Mothers day Questionnaire.
Is there a way to download the flower pot template with the flower ?
are the flowers paper diecuts or did you sponge paint them?
Hi there! They were diecuts! But the sponge paint idea would be so good!
Do you have something similar for the dad’s?
I love this questionnaire. Could you please forward it to me in black and white? I would love to have the children paint it.
Thanks!
Sherry
Do you have ‘my grandma’ version of this. I would love one please
I do not! I am so sorry! Maybe one day!
Do you happen to have a template for the flower pot and flower that picture is in?
I do not! I am so sorry! It is a die-cut from our teacher workroom in our preschool!
